Dental Insurance

Dental Plus is true dental insurance for individuals that will cover the cost of dental care above and beyond the plan member’s co-payment. There is no maximum to the amount of dental services that this plan will cover, nor are there any deductibles that need to be met. Your coverage gives you simple access to total dental care. Routine and preventative services are available to you at a low co-payment. Major services like crowns, bridges and dentures are provided at substantial savings with predictable costs.

Orthodontia through Willamette Dental offices is available with this plan for both adults and children after a six month waiting period. Plan participants do not need to fill out or submit claim forms nor do they need to deal with any complicated insurance forms. As a plan member, you simply schedule your appointments, see the dentist and pay at that visit. Willamette Dental makes access to quality dental care easy, while Dental Plus keeps that care affordable for you and your family.

How to Enroll

To enroll in the Dental Plus program, simply complete the entire application form and submit it along with payment. No physical examinations are required to be considered for coverage.

Your premiums may be paid either semi-annually or annually. Payment must be received by the 25th of the month preceding the renewal date or effective date for coverage to be active.

Family coverage may include a legal spouse and unmarried dependent children through age 24 provided the child receives more than one-half (1/2) of his or her financial support from the Member. Please note that dependent children are not eligible for single coverage.

Coverage is effective the first of the month, if you have enrolled by the 25th of the previous month.

To participate in the Dental Plus of Idaho Insurance plan you must be a resident of Idaho and receive your care from a Willamette Dental Office. An advance appointment is required to receive care. For your appointments, please call 1-800-603-1738.

Most offices are open Monday through Saturday, 7 AM to
6 PM. In making your appointments you can anticipate being seen as follows:
Emergencies: Within Approximately 24 Hours
for Pain, Bleeding, or Swelling
First Appointment: 30 Days
Regular Hygiene - Cleanings: 45 Days
Non-Emergency Operative: 60 Days
With the exception of emergencies, the number of days are averages. The length of wait-time for an appointment may vary based on your choice of dentist, dental office location, and your desired day or time of appointment.

When you speak to a Willamette Dental representative or arrive at the dental office for your appointment, simply identify yourself as a Dental Plus member. You will then receive dental care in accordance with your plan.

Costs

The cost of most preventative and restorative dental services are covered with a low co-payment per procedure. Major dental services are covered at typically 50% or less of Usual, Customary and Reasonable charges, which is the standard of fees for your area. To receive low cost dental plan services, charges must be paid in full at the time of
the visit.

Your provider reserves the right to determine payment schedules for extended treatment. Visa, MasterCard and Discover cards are accepted.

The benefits of this plan are ONLY offered when services are provided by a dentist or specialist at a Willamette Dental Office.

Is this plan offered to Washington residents?

You must be an Idaho resident to be eligible for Dental Plus of Idaho. However, the benefits of this plan are offered at all Willamette Dental offices in Oregon, Washington, and Idaho.

What can I expect at my first visit?

If you are a new Willamette Dental patient, your dentist will need to examine your current oral health in order to make an informed recommendation for a personalized treatment schedule. Your first visit will probably include a new patient exam and x-rays. Cleanings and other preventive procedures can be scheduled after this initial appointment.

How are payments made for Dental Plus of Idaho?

All co-pays and charges incurred for dental services are to be paid in full at the time of service.

How do I know how much Willamette Dental charges for dental procedures?

The cost of most preventative and restorative services are covered with a per procedure co-payment. Major services are covered at a significant reduction from Usual and Customary and Reasonable (UCR) Charges. Willamette Dental’s UCR Charges are very close to those you would find at most other dental practices in Idaho. Are there any waiting periods under this plan?

With the exception of complete orthodontia, there are no waiting periods with Dental Plus of Idaho. If your payment and application is received by the 25th of the month, then eligibility for Dental Plus of Idaho begins on the 1st of the following month.

Will I receive a Dental Plus insurance card when I become a member?

Yes. Your dental Plus ID card(s) will be mailed to you within 10 business days after receipt of premium payment. However, as long as you are a member of Dental Plus of Idaho, you do not need an ID card to be seen or to make an appointment.
